DESCRIPTION:Our monthly community social work and nursing clinic provides accessible\, patient-centered support for individuals in need. Services include referrals to primary care and substance use treatment\, care coordination\, and individualized support. The clinic also offers basic wound care\, vaccinations\, vitals monitoring\, and assistance with navigating local essential resources\, including General Assistance (GA)\, 3SquaresVT (food support)\, housing\, and disability vouchers. Our goal is to reduce barriers to care\, promote health equity\, and connect community members with the services they need to improve overall well-being.
